Dr. Jacobs graduated from the University of Iowa College of Medicine. He completed his residency at Memorial Hospital, South Bend, Indiana.

He is Board Certified in Family Medicine and earned his fourth Board Re-certification in 2016.

Dr. Jacobs also earned his Certificate of added qualifications in Primary Care Sports Medicine in 1999 and re-certified in 2009.

He is also a member of The American Academy of Family Practice and the American Medical Society for Sports Medicine.

Dr. Jacobs has been in practice since 1987.
