Family Medicine will begin administering the Covid Vaccine for ages 6 months – 4 years on June 27, 2022 by appointment only. If you have questions regarding this vaccine or wish to make an appointment, please contact our office at (574) 234-4016.

Family Medicine is no longer mandating patients to wear a mask at our facility. It is now optional as long as you are not having any upper respiratory symptoms. If you are experiencing these symptoms then you will be required to wear a mask.

Family Medicine is now offering Covid-19 testing for our patients.  Please call the office to schedule your testing appointment.  We are testing Monday – Friday from 8:30 a.m. – 10:30 a.m. behind our building.  You must have an appointment to be tested.  Please call (574) 234-4016 for scheduling.

We know many of our patients do not want to come into the office for medication refills, follow up appointments on chronic conditions, etc. so Family Medicine of South Bend will now be offering Telehealth visits via Skype. Please call our office at (574) 234-4016 to schedule your Telehealth visit. Please continue to visit our website at www.individualcareforyou.com for further updates. We appreciate your patience and understanding during this difficult time.
