After over 35 years of caring for our own inpatients at Memorial Hospital, we have decided to partner with Beacon Adult Hospitalists to collaboratively care for our inpatients at Memorial Hospital. This will take place effective October 1, 2024. Our new collaborative relationship with Beacon Adult Hospitalists will allow us to remain the navigators of our patient’s healthcare in our office-based practice.
This continues our evolution as an independent practice. Our patients continue to have a choice between both St. Joseph Hospital Mishawaka and Memorial Hospital of South Bend. We will collaborate with the emergency department physicians, hospitalists and specialists at both institutions.
Effective immediately, Family Medicine will no longer be participating in the VFC program. Our Medicaid, uninsured and underinsured pediatric patients will need to go to the health department for any needed vaccines.
We will begin our walk-in flu clinic on Tuesday, September 3, 2024. The hours of the walk-in flu clinic are 8:00 – 11:30 a.m. and 1:00 p.m. – 4:30 p.m. Monday-Friday. If you have a scheduled appointment, you can also get the flu vaccine at that time. Any questions regarding the flu vaccine, please send a message through the patient portal or call (574) 234-4016.
Family Medicine is excited to welcome Dr. Natalie Frentz to our practice on February 5, 2024. Dr. Frentz has been in practice since 2007 caring for all ages in family medicine.

In an effort to increase the efficiency of handling all prescription refill requests we are asking all patients to contact their pharmacy for all refill requests. If you are having trouble reaching your pharmacy, you can send a message through our patient portal. You can access the patient portal through our website at www.individualcareforyou.com. Even if you are out of refills, you can still contact your pharmacy and they will send the prescription refill request electronically to your physician.
This new process will eliminate the need for you to wait on hold to request a refill from one of our staff. Our goal is to refill your prescription within 48 hours Monday through Friday.
