Family Medicine will begin administering the Covid Vaccine for ages 6 months – 4 years on June 27, 2022 by appointment only. If you have questions regarding this vaccine or wish to make an appointment, please contact our office at (574) 234-4016.

Family Medicine is no longer mandating patients to wear a mask at our facility. It is now optional as long as you are not having any upper respiratory symptoms. If you are experiencing these symptoms then you will be required to wear a mask.

In an effort to increase the efficiency of handling all prescription refill requests we are asking all patients to contact their pharmacy for all refill requests.  If you are having trouble reaching your pharmacy, you can send a message through our patient portal.  You can access the patient portal through our website at www.individualcareforyou.com.  Even if you are out of refills, you can still contact your pharmacy and they will send the prescription refill request electronically to your physician.

This new process will eliminate the need for you to wait on hold to request a refill from one of our staff.  Our goal is to refill your prescription within 48 hours Monday through Friday.

HOOSIERS AGE 80 AND OLDER CAN REGISTER FOR COVID-19 VACCINE

INDIANAPOLIS — Gov. Eric J. Holcomb and the Indiana Department of Health today announced that Hoosiers age 80 and older will be eligible to register for a free COVID-19 vaccine beginning Friday, Jan. 8.

Individuals age 80 and older account for less than 4 percent of the state’s population but represent more than 19 percent of the hospitalizations and more than half of the COVID-19 deaths in the state, according to the Indiana Department of Health.

State Health Commissioner Kris Box, M.D., FACOG, said vaccine supplies are still limited. Indiana has received just over 350,000 doses of vaccine to date and is scheduled to receive about 78,000 vaccines per week at this time.

“By opening vaccine to those who are 80 or older, then adding people in their 70s and 60s when vaccine supplies allow us to expand, we can best protect the populations that account for 93 percent of our COVID-19 deaths,” Box said.

Hoosiers age 80 and older can register beginning at 9 a.m. Friday by going to ourshot.in.gov, searching for a nearby vaccine clinic and selecting an appointment time. Appointments may also be made by calling 211. A caregiver or loved one may make an appointment on behalf of an eligible senior.

At least one vaccine clinic will be located in each Indiana county.

Appointments for the second dose will be made at the clinic when the first dose is administered.

Due to the weather, Family Medicine will not be having anymore drive up flu clinics on Saturday. We still have plenty of flu vaccines available so please call to schedule a nurse visit to get your flu vaccine. We will not be doing walk-in flu shots. Family Medicine is currently not seeing patients in the office on Saturday; however, the office is open from 9:00 a.m. – noon for telephone calls and we are scheduling Telehealth visits for Saturday. Family Medicine is also still offering COVID-19 testing Monday-Friday. You must call to schedule an appointment at (574) 234-4016.
